What’s Your Score is an informal screening tool that also serves as a communication pathway for employees to gain access to multiple wellness services.
Employees will be classified into groups based upon survey results. Once classified contact will be made with the at risk respondents. Medium to high risk employees will be the primary focus of our attention. It should be noted that all respondents will receive appropriate attention.
What’s Your Score is our premier tool. This survey is cost effective and as an initial screening tool designed to identify and target employees who are classified as unhealthy. The ultimate goal is to get a snap shot of the overall risk level for the entire employee population. Respondents are classified into low/ medium/ and high risk categories dependent upon their answers to the survey. Classifications are based upon on their potential risk for heart disease, injury potential and overall activity level. This survey is the perfect complement to your HRA. What’s Your score will determine the perfect approach for each specific employee group active to non-active.

Health Risk Appraisals
After an initial interview with your key personnel we will help you choose an HRA that best fits your companies needs. Our goal is to help you find the perfect product for the size of your company. As your company changes so will the products that will serve you the best.
An assessment tool used by health promoters to evaluate a person's health. The appraisal usually takes the form of an extended questionnaire that enquires into personal lifestyle, and personal and family medical history. The appraisal may also include a physical examination, laboratory tests of blood chemistry (e.g. of cholesterol level), blood pressure, and physical fitness levels. The outcome is a profile identifying specific risks (e.g. heavy smoking and sedentary lifestyle) with strategies and targets for reducing the risks.
A health risk appraisal (HRA) is a computer-based health-promotion tool, consisting of a questionnaire, a formula for estimating health risks (e.g., premature death), an advice database, and a means to generate reports. Its roots lie in efforts to guide physicians in promoting health as well as treating illness. HRAs are widely used in worksite wellness programs and community-based health programs as well as health care settings.
Typical aims are to assess health risks and lifestyle choices; to motivate individuals to adopt or maintain beneficial lifestyle practices; to guide health-promotion activities of health plans, employers, and health agencies; and to create a database profile of the population appraised. A key element of any HRA is personalized feedback to identify areas of excess health risk and recommend actions
Stress Management
Stress is the wear and tear on our bodies when we are responding to daily activities. There are physical and emotional responces to stressful situations that create positive and negative impact upon our bodies. Positive stress helps us to excel in life; negative stress can stop you in your tracks. The negative influence of stress can result in feelings of rejection, anger, and depression.
